Amdocal 5 mg is a long-acting calcium channel blocker that helps relax blood vessels and improve blood flow, reducing blood pressure and alleviating angina. It may be used alone or in combination with other antihypertensive or antianginal agents for the treatment of hypertension and coronary artery disease. Amdocal 5 mg is effective in patients with chronic stable angina, vasospastic angina, and angiographically documented coronary artery disease without heart failure or reduced ejection fraction.

Amdocal 5 mg is indicated for:
Hypertension (high blood pressure)
Coronary artery disease (CAD), including:
Chronic stable angina
Vasospastic (Prinzmetal’s) angina
Angiographically documented CAD in patients without heart failure or ejection fraction <40%
Calcium-channel blockers
Amdocal 5 mg inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. By relaxing vascular smooth muscles, it reduces peripheral vascular resistance and lowers blood pressure. Its antianginal effect is attributed to:
Peripheral arteriole dilation, reducing afterload and myocardial oxygen consumption
Dilation of coronary arteries and arterioles, improving oxygen delivery in ischemic regions
These combined actions reduce total ischemic burden without significantly affecting heart rate.
Adults:
Starting dose: 5 mg once daily
Maximum dose: 10 mg once daily
Pediatric patients (6–17 years):
Starting dose: 2.5 mg once daily, titrated to 5 mg once daily if needed
Elderly, fragile, or hepatic impairment:
Starting dose: 2.5 mg once daily, titrate carefully
Administration tips:
Take at the same time each day
Can be administered with or without food
Compatible with thiazide diuretics, alpha blockers, beta blockers, ACE inhibitors, nitrates, sublingual nitroglycerin, NSAIDs, antibiotics, and oral hypoglycemics
Avoid grapefruit or grapefruit juice as it may increase bioavailability and blood pressure-lowering effects
Known hypersensitivity to Amdocal 5 mg or amlodipine
Severe aortic stenosis or cardiogenic shock (use with caution)
Common:
Headache
Peripheral edema (dose-related)
Fatigue
Nausea
Abdominal pain
Somnolence
Rare:
Palpitations, flushing, dizziness, rash, constipation, or diarrhea
Symptomatic hypotension is possible, especially in severe aortic stenosis
Worsening angina or myocardial infarction may occur after initiating or increasing dose in patients with severe CAD
Slow titration recommended in hepatic impairment
Monitor cardiovascular status in patients with severe comorbidities
Symptoms may include: excessive peripheral vasodilation, reflex tachycardia, hypotension, and shock. Management includes:
Cardiovascular support
Elevation of extremities and fluid management
Vasoconstrictors if indicated
Intravenous calcium gluconate may reverse calcium channel blockade effects
Activated charcoal may reduce absorption if given within 2 hours
Pregnancy Category C: Use only if benefits outweigh potential risks
Safety in breastfeeding not established; discontinue nursing if treatment is necessary
Store in a dry place away from light and heat.
Keep out of reach of children.
