Invasive aspergillosis
Candidemia and other serious Candida infections
Esophageal candidiasis
Scedosporium and Fusarium infections
Other severe systemic fungal infections, especially in immunocompromised patients
Prophylaxis of fungal infections in patients undergoing bone marrow or stem cell transplantation
Voriconazole is a triazole antifungal that works by inhibiting fungal cytochrome P450-dependent 14α-lanosterol demethylation, a critical step in ergosterol biosynthesis. Lack of ergosterol compromises the integrity of the fungal cell membrane, leading to increased permeability and cell death.
Voriconazole exhibits broad-spectrum activity against various yeasts and molds, including Candida species, Aspergillus species, Fusarium, and Scedosporium. Its excellent tissue penetration makes it effective for systemic and invasive infections, including those affecting the lungs, bloodstream, central nervous system, and other organs.
Adults: Typical oral dose is 200 mg every 12 hours for the first day (loading dose), followed by 100 mg every 12 hours. Dosage may vary depending on infection severity, patient weight, and liver function.
Children: Dosing is weight-based and should be determined by a specialist.
Tablets should be taken at least one hour before or one hour after meals to ensure optimal absorption.
Regular monitoring of liver function tests and plasma voriconazole levels is recommended for long-term therapy.
Hypersensitivity to Voriconazole or other azole antifungals
Concomitant use with drugs that are strongly contraindicated due to interactions (e.g., certain drugs metabolized by CYP3A4)
Common adverse effects may include:
Visual disturbances (blurred vision, altered color perception)
Nausea, vomiting, or diarrhea
Headache
Elevated liver enzymes
Skin rash
Serious adverse effects, although rare, may include hepatotoxicity, arrhythmias, and severe hypersensitivity reactions. Immediate medical attention is required if severe reactions occur.
Voriconazole is not recommended during pregnancy unless benefits outweigh risks.
Nursing mothers should avoid use, or discontinue breastfeeding during therapy due to potential drug excretion in breast milk.

Avoid excessive sunlight as photosensitivity reactions may occur
