Lacomax 100 mg contains Lacosamide, an antiepileptic drug used to manage partial-onset seizures in adults and pediatric patients aged 4 years and above. Antiepileptic / Anticonvulsant
Lacosamide exerts its antiepileptic effects through selective enhancement of slow inactivation of voltage-gated sodium channels, stabilizing hyperexcitable neuronal membranes and inhibiting repetitive neuronal firing. Unlike traditional sodium channel blockers, Lacosamide does not significantly affect the fast inactivation process, which contributes to its efficacy and tolerability.
Pharmacokinetic properties of Lacosamide include:
Rapid and almost complete oral absorption, with peak plasma concentrations reached within 1–4 hours
Bioavailability close to 100%, unaffected by food
Low protein binding (less than 15%), minimizing drug-drug interactions
Metabolism occurs primarily via CYP2C19 and minor CYP-independent pathways
Elimination half-life is approximately 13 hours, allowing for twice-daily dosing
Excretion is mainly through the kidneys, with around 40% of the drug excreted unchanged
Lacomax 100 mg is indicated for:
Adjunctive therapy in adults and children (≥4 years) with partial-onset seizures
Monotherapy for partial-onset seizures in adults with newly diagnosed epilepsy
Adults:
Initial dose: 50 mg twice daily
Titration: Increase by 100 mg/day weekly up to a maintenance dose of 200–400 mg/day (divided into two doses)
Maximum recommended dose: 400 mg/day
Pediatric Patients (≥4 years):
Initial dose: 1–2 mg/kg twice daily
Titration: Gradually increase by 1–2 mg/kg every week
Maximum recommended dose: 12 mg/kg/day or 400 mg/day, whichever is lower
Administration Notes:
Can be taken with or without food
Tablets should be swallowed whole with water
Doses should be adjusted gradually to minimize the risk of adverse effects
Known hypersensitivity to Lacosamide or any excipients in the formulation
Caution is advised in patients with severe cardiac conditions, including conduction abnormalities
Lacosamide is generally well tolerated. Common adverse reactions include:
Dizziness, headache, and fatigue
Nausea and vomiting
Diplopia or blurred vision
Rare cardiac effects such as PR interval prolongation, arrhythmias, or syncope
Ataxia and tremor in some patients
Monitor ECG in patients with pre-existing conduction abnormalities or those on other drugs affecting cardiac conduction
Dose adjustments may be necessary in patients with renal or hepatic impairment
Caution when used in combination with other central nervous system depressants
Lacosamide should be used during pregnancy only if clearly needed
Excretion into breast milk has been reported; nursing mothers should exercise caution
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
