By reducing acid secretion, Maxima 20 mg helps minimize mucosal irritation and supports long-term gastrointestinal health.
The recommended dosage depends on the condition being treated:
Symptomatic GERD: 20 mg once daily for 4 weeks
Healing of Erosive Esophagitis: 20 mg once daily for 4–8 weeks
Maintenance Therapy: 20 mg once daily
Duodenal Ulcer: 20 mg once daily for 2–4 weeks
Gastric Ulcer: 20 mg once daily for 4–8 weeks
H. pylori Eradication: 20 mg or 40 mg once daily in combination with appropriate antibiotics
The tablet should be taken at least one hour before meals and swallowed whole with water. Do not crush or chew the tablet.
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme system (proton pump) in gastric parietal cells. This enzyme is responsible for the final step in acid production. By blocking this mechanism, esomeprazole suppresses both basal and stimulated acid secretion.
After oral administration, peak plasma concentration occurs approximately 1.5 hours later. The drug is about 97% bound to plasma proteins and is metabolized in the liver primarily via CYP2C19 and CYP3A4 enzymes. The elimination half-life is approximately 1–1.5 hours. Around 80% of the administered dose is excreted in urine as inactive metabolites.
Esomeprazole may:
Reduce absorption of drugs dependent on gastric pH (e.g., ketoconazole, iron salts, digoxin)
Reduce clearance of diazepam
Show increased levels when used with clarithromycin
Have minimal clinically significant interaction with phenytoin, warfarin, quinidine, or oral contraceptives
Maxima Tablet 20 mg is contraindicated in patients with:
Known hypersensitivity to esomeprazole
Allergy to substituted benzimidazoles
Common side effects include:
Headache
Diarrhea
Nausea

Constipation
Flatulence
Dry mouth
Most adverse reactions are mild and self-limiting.
Symptom relief does not rule out gastric malignancy.
Use caution in severe hepatic impairment.
Long-term use may be associated with vitamin B12 deficiency or hypomagnesemia.
Not recommended in pediatric patients unless advised by a specialist.
Adequate human studies are lacking. Use during pregnancy only if clearly necessary. Breastfeeding should be discontinued if therapy is essential.
