Neofloxin 0.3% is a broad-spectrum fluoroquinolone antibacterial preparation containing ciprofloxacin, designed for the effective treatment of a wide range of bacterial infections. Owing to its strong bactericidal activity, excellent tissue penetration, and high serum and urinary concentrations, Neofloxin 0.3% is suitable for managing both single and mixed infections caused by susceptible organisms. It is particularly useful in infections where resistance to other antibiotics such as penicillins, cephalosporins, or aminoglycosides is suspected.

Neofloxin 0.3% may be used alone as empirical therapy while awaiting culture and sensitivity results or in combination with other antibiotics in severe, complicated, or life-threatening infections. Its enhanced antibacterial and antipseudomonal activity makes it a valuable option in hospitalized, immunocompromised, or critically ill patients.

Indications

Neofloxin 0.3% is indicated for the treatment of infections caused by sensitive bacteria, including:

  • Severe systemic infections: Septicaemia, bacteraemia, peritonitis, infected burns, and infections in immunosuppressed patients

  • Respiratory tract infections: Lobar pneumonia, bronchopneumonia, acute and chronic bronchitis, bronchiectasis, empyema, acute exacerbation of cystic fibrosis

  • Urinary tract infections: Urethritis, cystitis, pyelonephritis, prostatitis, epididymitis (both uncomplicated and complicated)

  • Skin and soft tissue infections: Infected ulcers, wound infections, abscesses, cellulitis, erysipelas, otitis externa, infected burns

  • Gastrointestinal infections: Enteric fever, infective diarrhea

  • Biliary tract infections: Cholangitis, cholecystitis, gall bladder empyema

  • Intra-abdominal infections: Peritonitis, intra-abdominal abscesses

  • Bone and joint infections: Osteomyelitis, septic arthritis

  • Pelvic infections: Salpingitis, endometritis, pelvic inflammatory disease

  • ENT infections: Otitis media, sinusitis, mastoiditis, tonsillitis

  • Gonorrhoea: Urethral, rectal, and pharyngeal gonorrhoea, including beta-lactamase-producing strains

Pharmacology

Ciprofloxacin is a synthetic fluoroquinolone with potent bactericidal activity against a wide range of gram-positive and gram-negative organisms. It acts by inhibiting bacterial DNA synthesis through binding to DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, and repair. This mechanism results in rapid bacterial cell death.

After oral administration, ciprofloxacin is rapidly absorbed from the gastrointestinal tract and widely distributed into body tissues and fluids. Approximately 30–50% of the administered dose is excreted unchanged in the urine within 24 hours, making it especially effective in urinary tract infections.

Dosage and Administration

Dosage depends on infection severity, organism sensitivity, and patient factors such as age and renal function.

  • Adults: 100–750 mg twice daily depending on infection type

  • Urinary tract infections: 250–500 mg twice daily

  • Respiratory tract infections: 250–500 mg twice daily; up to 750 mg in severe cases

  • Gonorrhoea: Single dose of 250–500 mg

  • Cystic fibrosis (pseudomonal infection): 750 mg twice daily

Dose adjustment may be required in severe renal impairment. Neofloxin 0.3% may be taken with or without food, but adequate fluid intake is recommended.


Contraindications

  • Hypersensitivity to ciprofloxacin or other quinolones


Side Effects

Common side effects include gastrointestinal upset such as nausea, vomiting, diarrhea, and abdominal pain. Central nervous system effects such as headache, dizziness, confusion, or seizures may occur. Rare reactions include hypersensitivity reactions, photosensitivity, tendon or joint pain, hypoglycemia, and transient elevation of liver enzymes.


Pregnancy and Lactation

Neofloxin 0.3% should be used during pregnancy only if the potential benefit outweighs the risk. As ciprofloxacin is excreted in breast milk and may cause articular damage, its use during lactation is not recommended.
