Indications

Pharmacology / Mode of Action

Nitroglycerin, the active ingredient of Nitrosol SR Tablet 2.6 mg, is converted in the body to nitric oxide (NO). Nitric oxide activates guanylate cyclase, increasing intracellular levels of cyclic guanosine 3’,5’ monophosphate (cGMP). Elevated cGMP causes relaxation of vascular smooth muscle through dephosphorylation of myosin light chains, resulting in vasodilatation.

Venous dilation predominates, leading to reduced venous return (preload) and decreased myocardial oxygen demand. Arterial dilation reduces systemic vascular resistance (afterload), further lowering cardiac workload. Additionally, dilation of coronary arteries improves blood flow to ischemic areas of the myocardium, helping to prevent anginal symptoms.

The sustained-release formulation ensures gradual and prolonged drug release, providing extended protection against angina.

Dosage & Administration

Dosage should be individualized according to patient response and clinical condition.

Drug Interactions

Contraindications

• Hypersensitivity to nitroglycerin or other nitrates

• Severe anemia

• Increased intracranial pressure

• Hypotension or circulatory shock

• Concomitant use of phosphodiesterase-5 inhibitors

Side Effects

Common side effects may include:

• Headache

• Dizziness or light-headedness

• Flushing

• Hypotension

• Palpitations

Headache is the most frequently reported adverse effect and is usually dose-related.

Pregnancy & Lactation

Nitrosol SR Tablet 2.6 mg should be used during pregnancy only if clearly needed and prescribed by a physician. It is not known whether nitroglycerin is excreted in human breast milk; caution is advised during lactation.

Precautions & Warnings

• Use the lowest effective dose to minimize tolerance
