Penomer 1 gm/vial Injection contains Meropenem, a broad-spectrum carbapenem antibiotic belonging to the therapeutic class of other beta-lactam antibiotics. It is designed for parenteral use and is highly effective against a wide range of Gram-positive, Gram-negative, aerobic, and anaerobic bacteria. Penomer is commonly used in hospital settings for the treatment of severe and life-threatening bacterial infections where broad antimicrobial coverage is required.

Penomer 1 gm/vial is indicated for the treatment of infections in adults and children caused by single or multiple organisms sensitive to Meropenem, including:
Pneumonia and nosocomial pneumonia
Urinary tract infections
Intra-abdominal infections
Gynaecological infections such as endometritis and pelvic inflammatory disease
Skin and skin structure infections
Meningitis
Septicaemia
Pulmonary infections in cystic fibrosis
Empiric treatment of presumed infections in patients with febrile neutropenia
Meropenem exerts a rapid bactericidal effect by inhibiting bacterial cell wall synthesis. It penetrates bacterial cell walls efficiently and shows strong affinity for penicillin-binding proteins (PBPs). Its high stability against serine beta-lactamases makes it effective against many resistant organisms, providing reliable coverage in severe infections.
The dosage and duration of therapy depend on the type and severity of infection and the patient’s clinical condition.
Adults: 500 mg to 1 g intravenously every 8 hours. Higher doses (up to 2 g every 8 hours) may be used in conditions such as meningitis and cystic fibrosis.
Children (3 months to 12 years): 10–40 mg/kg IV every 8 hours depending on infection severity.
Children weighing over 50 kg should receive adult doses.
Penomer may be administered as an intravenous infusion over 15–30 minutes or as an intravenous bolus over 3–5 minutes.
Probenecid may increase plasma concentrations of Meropenem by reducing renal excretion. Meropenem can also reduce serum levels of valproic acid, potentially leading to sub-therapeutic concentrations.
Penomer is contraindicated in patients with known hypersensitivity to Meropenem or other beta-lactam antibiotics.
Penomer is generally well tolerated. Possible adverse effects include pain or inflammation at the injection site, thrombophlebitis, skin reactions, gastrointestinal disturbances, headache, and diarrhea.

Renal impairment: Dose adjustment is required when creatinine clearance is below 51 ml/min.
Hepatic impairment: No dosage adjustment is necessary.
Elderly: Dose adjustment only if renal function is impaired.
Pregnancy and lactation: Pregnancy Category B; use only if clearly needed and with caution during breastfeeding.
Pediatric use: Safety in infants under 3 months has not been established.
Store the vial below 30°C in a cool, dry place, protected from light and moisture. Reconstituted solutions should be used promptly; however, stability is maintained for limited periods under recommended conditions.
