Indications

Penomer Injection 500 mg/vial is indicated for the treatment of infections caused by single or multiple organisms sensitive to meropenem, including:

• Pneumonia and nosocomial pneumonia

• Urinary tract infections

• Intra-abdominal infections

• Gynecological infections such as endometritis and pelvic inflammatory disease

• Skin and skin structure infections

• Meningitis

• Septicaemia

• Pulmonary infections in cystic fibrosis

• Empiric treatment of presumed infections in patients with febrile neutropenia

Pharmacology

Meropenem is a carbapenem antibiotic with potent bactericidal activity. It inhibits bacterial cell wall synthesis by binding to essential PBPs. The drug is highly resistant to degradation by serine beta-lactamases, contributing to its effectiveness against resistant organisms. Meropenem demonstrates rapid onset of action and maintains effective plasma concentrations suitable for treating severe systemic infections.

Dosage and Administration

Dosage and duration depend on the type and severity of infection and the patient’s clinical condition.

Adults:

• Usual dose: 500 mg to 1 g intravenously every 8 hours

• Severe infections such as nosocomial pneumonia, septicaemia, and neutropenia: 1 g IV every 8 hours

• Meningitis: 2 g IV every 8 hours

• Cystic fibrosis: Up to 2 g IV every 8 hours

Children:

• 3 months to 12 years: 10–40 mg/kg IV every 8 hours

• Meningitis: 40 mg/kg IV every 8 hours

• Febrile neutropenia: 20 mg/kg IV every 8 hours

• Children weighing over 50 kg should receive adult doses

Penomer Injection may be administered as an intravenous infusion over 15–30 minutes or as an IV bolus over 3–5 minutes.

Contraindications

Penomer Injection is contraindicated in patients with known hypersensitivity to meropenem or other beta-lactam antibiotics.

Adverse Effects

Penomer Injection is generally well tolerated. Possible side effects include:

• Injection site inflammation or pain

• Thrombophlebitis

• Skin reactions such as rash, pruritus, or urticaria

• Gastrointestinal symptoms including nausea, vomiting, diarrhea, and abdominal pain

• Headache

Use in Special Populations

• Renal impairment: Dose adjustment is required

• Hepatic impairment: No dose adjustment necessary

• Elderly: Dose adjustment only if renal function is impaired

• Pregnancy: Category B; use only if clearly needed

• Lactation: Caution advised
