Primace Tablet 10 mg is indicated for:
Hypertension: Alone or in combination with other antihypertensives to achieve optimal blood pressure control.
Heart Failure: Management of symptomatic heart failure to improve survival and reduce hospitalizations.
Post-Myocardial Infarction: Reduces mortality and prevents progression of heart failure after a heart attack.
Cardiovascular Risk Reduction: Lowers the risk of stroke, myocardial infarction, and cardiovascular death in high-risk patients, including those with diabetes or established vascular disease.
Renal Protection: Slows progression of diabetic nephropathy and proteinuria by reducing intraglomerular pressure.
Angiotensin-Converting Enzyme (ACE) Inhibitor
Ramipril is a prodrug that is converted in the liver to its active form, Ramiprilat, which selectively inhibits the angiotensin-converting enzyme. This prevents the formation of angiotensin II, leading to:
Vasodilation
Reduced aldosterone secretion
Natriuresis and diuresis
Lower blood pressure and reduced cardiac preload and afterload
Additionally, Ramipril improves endothelial function, reduces vascular inflammation, and protects organs such as the heart and kidneys from long-term hypertensive damage. Antihypertensive effects are typically observed within one week, with maximal efficacy reached after 3–6 weeks.
Hypertension: Initial dose: 2.5–5 mg once daily; may be titrated to 10 mg once daily depending on response.
Heart Failure/Post-MI: Start with 1.25–2.5 mg once daily, gradually titrated as tolerated.
Renal or hepatic impairment: Dose adjustments may be necessary.
The tablet should be taken orally at the same time each day, with or without food, to maintain consistent blood levels.
Known hypersensitivity to Ramipril or other ACE inhibitors
History of angioedema related to previous ACE inhibitor therapy
Pregnancy, especially during the second and third trimesters
Bilateral renal artery stenosis or severe renal impairment
Common side effects include:

Dizziness, hypotension, or fatigue

Ramipril is contraindicated during pregnancy, particularly in the second and third trimesters, because of the risk of fetal injury. Limited information is available regarding excretion in human milk; breastfeeding is not recommended during therapy.
