Used alone or in combination with other antihypertensive agents such as thiazide diuretics to control high blood pressure.
Renal Protection in Type-2 Diabetic Patients with Proteinuria:
Recommended for hypertensive type-2 diabetic patients with proteinuria (urine albumin to creatinine ratio >300 mg/g) to delay the progression of renal disease.
Prosan 25 mg is a non-peptide, orally active angiotensin II receptor blocker. It selectively binds to the AT1 receptor found in tissues such as the heart, kidneys, adrenal glands, and vascular smooth muscle. By blocking the effects of angiotensin II, it inhibits vasoconstriction and reduces aldosterone secretion, leading to lowered blood pressure and improved renal protection.
Prosan 25 mg blocks the binding of angiotensin II to the AT1 receptor, preventing vasoconstriction and aldosterone-mediated sodium and water retention. This results in vasodilation, reduced blood pressure, and decreased strain on the cardiovascular and renal systems.
The usual starting and maintenance dose for most patients is 50 mg once daily.
If adequate blood pressure control is not achieved, 25 mg twice daily may be considered before increasing the dose.
In patients with intravascular volume depletion (e.g., those on high-dose diuretics), a starting dose of 25 mg once daily is recommended.
Prosan 25 mg may be administered once or twice daily.
The total daily dose ranges from 25 mg to 100 mg, depending on patient response and clinical condition.
Rifampicin and fluconazole may reduce levels of the active metabolite of Prosan 25 mg.
Concomitant use with hydrochlorothiazide may enhance antihypertensive effects.
Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes may increase the risk of hyperkalemia.
NSAIDs such as indomethacin may reduce the antihypertensive effect.
Combined use of an ACE inhibitor, ARB, NSAID, and thiazide diuretic may increase the risk of renal impairment.

Prosan 25 mg is generally well tolerated. Most adverse effects are mild and transient, including:
Dizziness
Diarrhea
Nasal congestion
Cough
Upper respiratory tract infections
Less common effects may include fatigue, edema, chest pain, nausea, headache, and pharyngitis.
Pregnancy: Category D. Use during the second and third trimesters increases the risk of fetal injury and should be avoided.
Lactation: It is not known whether Prosan 25 mg is excreted in human milk. A decision should be made to discontinue breastfeeding or the drug, considering the importance of therapy to the mother.
Use during pregnancy may cause fetal and neonatal morbidity and mortality due to reduced fetal renal function.
Symptomatic hypotension may occur in volume-depleted patients.
Patients with hepatic impairment may have increased plasma concentrations.
Changes in renal function, including renal failure, have been reported in patients with renal impairment.
Store in a cool, dry place away from heat and light.
