Remmo 40 mg is a potent proton pump inhibitor (PPI) formulated to provide effective control of gastric acid-related disorders. It contains Esomeprazole, the S-isomer of omeprazole, which specifically inhibits the H⁺/K⁺-ATPase enzyme in gastric parietal cells. By reducing both basal and stimulated gastric acid secretion, Remmo 40 mg promotes healing of erosive esophagitis, alleviates symptoms of gastroesophageal reflux disease (GERD), and helps prevent recurrence of duodenal and gastric ulcers.

Remmo 40 mg is also used as part of triple therapy for the eradication of Helicobacter pylori in duodenal ulcer patients and is indicated in Zollinger-Ellison syndrome and other pathological hypersecretory conditions. The medication is available in oral capsules, delayed-release suspension, and intravenous formulations for flexible administration.

Mode of Action

Esomeprazole in Remmo 40 mg selectively inhibits the gastric proton pump (H⁺/K⁺-ATPase), which is the final step in acid secretion. This results in a rapid and sustained reduction of gastric acidity. The S-isomer provides enhanced acid control compared to racemic PPIs. Remmo 40 mg is absorbed as enteric-coated pellets, with peak plasma concentrations occurring approximately 1.5 hours post-dose. It is extensively metabolized in the liver via CYP2C19 and CYP3A4, and less than 1% is excreted unchanged in the urine.

Indications

Remmo 40 mg is indicated for:

• Relief of GERD-related symptoms, including persistent heartburn

• Treatment of erosive esophagitis and maintenance of healing

• Helicobacter pylori eradication as part of combination therapy (with amoxicillin and clarithromycin) to reduce duodenal ulcer recurrence

• Zollinger-Ellison syndrome and other hypersecretory conditions

• Acid-related dyspepsia

• Duodenal and gastric ulcers

Dosage and Administration

Oral Capsules (Adults):

• Erosive esophagitis healing: 20–40 mg once daily for 4–8 weeks; may extend 4–8 weeks if needed

• Maintenance of healed erosive esophagitis: 20 mg once daily

• Symptomatic GERD: 20 mg once daily for 4 weeks; may extend another 4 weeks if needed

• Helicobacter pylori eradication (Triple therapy): Esomeprazole 40 mg once daily, amoxicillin 1000 mg twice daily, clarithromycin 500 mg twice daily for 10 days

• Zollinger-Ellison syndrome: 20–80 mg once daily; adjust individually

• Acid-related dyspepsia: 20–40 mg once daily for 2–4 weeks

• Duodenal ulcer: 20 mg once daily for 2–4 weeks

• Gastric ulcer: 20–40 mg once daily for 4–8 weeks

Oral Delayed-Release Suspension:

• Mix 15 ml water with the contents, allow 2–3 minutes to thicken, stir and consume within 30 minutes. Can also be administered via nasogastric or gastric tube.

Intravenous Injection:

• 40 mg IV over at least 3 minutes or 10–30 minutes infusion. Should not be mixed with other medications at the same site.

Administration Notes:

• Take capsules at least 1 hour before meals.

• Capsules may be opened and pellets mixed with cold applesauce if swallowing is difficult; consume immediately.

Contraindications

• Hypersensitivity to esomeprazole or any component of the formulation

Side Effects

Common side effects include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth. Adverse effects during long-term maintenance therapy are similar in type to short-term treatment.

• Do not assume symptom improvement rules out gastric malignancy

• Monitor patients with severe hepatic impairment (max 20 mg daily)
