Twincard is a fixed-dose combination antihypertensive medication containing Atenolol and Chlorthalidone. It is used for the treatment of hypertension (high blood pressure). Effective blood pressure control significantly reduces the risk of both fatal and non-fatal cardiovascular events, including stroke and myocardial infarction.

Combined antihypertensive preparations
Twincard combines two well-established antihypertensive agents that act through complementary mechanisms:
Atenolol is a cardioselective beta₁-adrenergic receptor blocker. It reduces heart rate, cardiac output, and blood pressure by blocking sympathetic stimulation of the heart.
Chlorthalidone is a long-acting thiazide-like diuretic. It promotes diuresis by increasing the excretion of sodium and chloride at the distal convoluted tubule of the nephron, leading to a reduction in plasma volume and blood pressure.
Clinical studies have demonstrated that Atenolol and Chlorthalidone do not interfere with each other’s bioavailability when administered together, making this combination a convenient and effective option for hypertension management.
Initial recommended dose: Atenolol 50 mg and Chlorthalidone 25 mg once daily
If blood pressure is not adequately controlled, an additional antihypertensive agent may be added gradually, starting at 50% of the usual initial dose to avoid excessive hypotension
Dose adjustment may be required in patients with severe renal impairment, as Atenolol is primarily excreted by the kidneys
Use in elderly: Clinical data are limited in patients aged 65 years and above; careful monitoring is advised
Concomitant use with digitalis glycosides may increase the risk of bradycardia due to additive effects on atrioventricular conduction and heart rate
Twincard is contraindicated in patients with:
Hypersensitivity to Atenolol, Chlorthalidone, or sulfonamide-derived drugs
Sinus bradycardia
Heart block greater than first degree
Cardiogenic shock
Overt cardiac failure
Anuria
The adverse effects of Twincard are consistent with those of its individual components.
Atenolol-related side effects may include:
Bradycardia
Cold extremities
Postural hypotension
Dizziness, vertigo, light-headedness
Fatigue, lethargy, drowsiness
Depression, sleep disturbances
Gastrointestinal discomfort such as diarrhea and nausea
Wheeziness or dyspnea
Chlorthalidone-related side effects may include:
Orthostatic hypotension
Anorexia, gastric irritation, vomiting
Constipation, abdominal cramping
Vertigo
Skin reactions such as rash, urticaria, photosensitivity
Metabolic changes including hyperglycemia, glycosuria, hyperuricemia
Muscle spasm, weakness, restlessness
Pregnancy Category D
Use during pregnancy is not recommended unless clearly necessary
Caution should be exercised when administering Twincard to breastfeeding women
Twincard may worsen peripheral arterial circulatory disorders
Regular monitoring of serum electrolytes is recommended to detect possible electrolyte imbalance
Caution is advised in patients with renal impairment and in elderly patients
There is no specific information on Twincard overdose in humans. Management is supportive and symptomatic and may include:
Removal of unabsorbed drug by induced emesis or activated charcoal
Hemodialysis to remove Atenolol from circulation
Correction of hypotension, dehydration, and electrolyte imbalance using standard medical procedures
Store in a cool and dry place
Protect from light.
