It is particularly beneficial in conditions where reduced blood flow, hypoxia, and metabolic disturbances of the brain play a key role. Vinsetine improves cerebral metabolism and microcirculation, thereby enhancing brain function and neurological recovery.

Pharmacology

Vinsetine 5 mg acts by significantly increasing cerebral metabolism and improving the utilization of glucose and oxygen in brain tissue. It enhances cerebral tolerance to hypoxia by shifting glucose metabolism toward the more energy-efficient aerobic pathway while also supporting anaerobic metabolism when required. Vinsetine increases adenosine triphosphate (ATP) concentration and improves the ATP/AMP ratio in the brain, which is essential for neuronal energy balance.

In addition, Vinsetine elevates the levels of key neurotransmitters such as norepinephrine, dopamine, and serotonin, contributing to improved cognitive and neurological function. It markedly improves cerebral microcirculation by inhibiting platelet aggregation, reducing abnormally increased blood viscosity, and increasing erythrocyte deformability. This promotes better oxygen delivery to brain tissues by lowering the oxygen-binding affinity of red blood cells.

Vinsetine selectively increases cerebral blood flow and enhances the proportion of cardiac output directed to the brain without significantly affecting systemic circulation, including blood pressure and heart rate. Importantly, it does not cause a steal phenomenon; instead, it preferentially improves blood flow to ischemic and injured brain areas while maintaining circulation in healthy regions, a phenomenon known as the inverse steal effect.

Indications

Vinsetine 5 mg is indicated for a wide range of conditions, including:

Acute Cerebrovascular Disorders

• Ischaemic stroke due to cerebral thrombosis or embolism

• Acute circulatory disorders and hypertensive crisis

• Ischaemic neurological deficit and complete stroke

• Multi-infarct dementia

• Cerebral arteriosclerosis

• Hypertensive encephalopathy

• Post-apoplectic conditions, including those following haemorrhagic stroke

Senile Disorders

• Memory impairment and forgetfulness

• Reduced concentration and slow thinking

• Dizziness and mood instability

• Sleep disturbances and aphasia

• Vasovegetative symptoms of menopausal syndrome

Visual Disorders

• Vascular disorders of the retina and choroid

• Arteriosclerosis-related visual impairment

• Vasospasm, macular degeneration

• Retinal arterial or venous thrombosis or embolism

• Secondary glaucoma due to vascular causes

Hearing Disorders

• Hearing impairment of vascular or toxic origin

• Presbyacusis

• Ménière’s disease

• Cochleovestibular neuritis

• Tinnitus and dizziness of labyrinthine origin

Dosage and Administration

Vinsetine 5 mg tablets are usually administered at a total daily dose of 15–30 mg in three divided doses with meals. The maintenance dose is typically 15 mg daily, given as 5 mg three times a day for long-term therapy.

In acute conditions, parenteral treatment may be initiated before switching to oral therapy, as directed by a physician.

Drug Interactions

Vinsetine injection is chemically incompatible with heparin and should not be mixed in the same syringe.

Contraindications

Parenteral use of Vinsetine is contraindicated in:

• Severe ischemic heart disease

• Severe cardiac rhythm disorders

• Pregnancy and lactation

Side Effects

Vinsetine is generally well tolerated. Occasionally, transient hypotension and tachycardia may occur, particularly with parenteral administration.

Precautions and Warnings

In acute stages of cerebrovascular disease, parenteral therapy is recommended until clinical improvement, followed by oral maintenance therapy. In chronic conditions, oral treatment alone is appropriate.
