Penicillinase-resistant Penicillins
Inclox 250 mg is indicated for infections caused by susceptible Gram-positive organisms, including:
Skin and Soft Tissue Infections
Boils, abscesses, carbuncles
Eczema, acne, furunculosis, cellulitis
Infected wounds and burns
Respiratory Tract Infections
Pneumonia, lung abscesses, empyema
Sinusitis, pharyngitis, tonsillitis, quinsy
Other Infections
Osteomyelitis, endocarditis
Urinary tract infections
Meningitis, septicaemia
Prophylactic Use
Used in major surgical procedures, particularly cardiothoracic and orthopedic surgeries, to prevent postoperative infections.
Flucloxacillin works by inhibiting bacterial cell wall synthesis, which is essential for bacterial survival. As an isoxazolyl penicillin, it is resistant to penicillinase enzymes produced by certain staphylococci, ensuring its effectiveness against penicillin-resistant strains.
It interferes with peptidoglycan cross-linking in the bacterial cell wall by inhibiting the transpeptidase enzyme. This results in weakened cell walls, swelling, and eventual bacterial lysis. Its acyl side chain prevents β-lactam ring hydrolysis by penicillinase, maintaining its bactericidal activity. Flucloxacillin has limited activity against Gram-negative bacilli.
Oral Administration
Adults: 250 mg four times daily; may be increased in severe infections
Osteomyelitis/Endocarditis: Up to 8 g/day in 6–8 hourly divided doses
Children 2–10 years: Half of adult dose
Children <2 years: Quarter of adult dose
Take oral doses 1 hour before meals
Parenteral Administration
Adults/Elderly: 250 mg four times daily IM; IV 250 mg–1 g four times daily via slow injection or infusion
Children: Half or quarter of adult dose depending on age
Severe infections may require doubling the dose
Special Routes
Intra-articular: 250–500 mg/day in 0.5% lignocaine
Intrapleural: 250 mg/day
Nebulization: 125–250 mg in 3 ml sterile water four times daily
Hypersensitivity to penicillins or flucloxacillin
Common adverse effects include:
Gastrointestinal: Nausea, vomiting, diarrhea, dyspepsia
Dermatological: Rash, urticaria, purpura
Hepatic: Cholestatic jaundice, hepatitis
Renal: Interstitial nephritis
Systemic: Fever
Dose adjustments may be necessary in severe renal failure (creatinine clearance <10 ml/min)
Pregnancy Category B – Use only if clearly necessary
Excreted in breast milk; caution advised in lactating mothers
Use with caution in patients with liver dysfunction or allergic predisposition
Monitor renal function in severe renal impairment
Store in a cool, dry place away from light and heat
Keep out of reach of children.
