Janmet XR Tablet 50 mg + 500 mg is an extended-release combination oral antidiabetic medication containing Sitagliptin 50 mg and Metformin Hydrochloride 500 mg (Extended Release). It is indicated for the management of Type 2 Diabetes Mellitus in adults, particularly in patients who require improved glycemic control with the added convenience of once-daily dosing. The extended-release (XR) formulation of Metformin allows gradual drug release, improving gastrointestinal tolerability and maintaining stable blood glucose levels throughout the day.
Type 2 diabetes is a chronic metabolic disorder characterized by insulin resistance, impaired insulin secretion, and increased glucose production by the liver. If left uncontrolled, it can lead to long-term complications such as cardiovascular disease, kidney failure, neuropathy, and vision problems. Janmet XR provides dual-action glucose control by targeting multiple underlying mechanisms of hyperglycemia.

Each extended-release tablet contains:
Sitagliptin 50 mg
Metformin Hydrochloride 500 mg (Extended Release)
Janmet XR works through complementary mechanisms:
Sitagliptin is a DPP-4 inhibitor that increases levels of incretin hormones (GLP-1 and GIP). These hormones stimulate glucose-dependent insulin release and suppress glucagon secretion, thereby reducing postprandial blood glucose levels.
Metformin (Extended Release) reduces hepatic glucose production (gluconeogenesis), decreases intestinal glucose absorption, and improves peripheral insulin sensitivity. The XR formulation ensures gradual release and prolonged action, minimizing peak-related gastrointestinal side effects.
Together, these agents effectively control both fasting and post-meal blood sugar levels with improved tolerability.
Janmet XR Tablet 50 mg + 500 mg is indicated for:
Management of Type 2 Diabetes Mellitus in adults
Patients inadequately controlled with Metformin alone
Patients already taking Sitagliptin and Metformin separately
As part of combination therapy with other antidiabetic medications when necessary
It is not indicated for Type 1 diabetes or diabetic ketoacidosis.
Janmet XR is usually taken once daily with the evening meal, or as directed by a physician. The tablet should be swallowed whole and not crushed, chewed, or split, as this may affect the extended-release properties.
Regular monitoring of blood glucose levels and HbA1c is recommended during treatment.
Once-daily dosing for better patient compliance
Extended-release Metformin for improved gastrointestinal tolerance
Effective control of fasting and postprandial blood glucose
Low risk of hypoglycemia when used alone
Convenient fixed-dose combination therapy
Assess renal function before initiating therapy and periodically thereafter
Use cautiously in patients with liver disease
Risk of rare but serious lactic acidosis with Metformin
Inform healthcare provider of all concurrent medications
Use during pregnancy or breastfeeding only under medical advice
Common side effects may include:
Nausea, diarrhea, abdominal discomfort
Headache
Upper respiratory tract symptoms
Hypoglycemia (more likely when used with insulin or sulfonylureas)
Seek medical attention if severe symptoms such as persistent vomiting, muscle pain, unusual tiredness, or allergic reactions occur.
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
