Angiotensin II is formed from angiotensin I through a reaction catalyzed by angiotensin-converting enzyme (ACE). It is the principal pressor hormone of the renin-angiotensin-aldosterone system and causes vasoconstriction, aldosterone release, cardiac stimulation, and renal sodium retention, all of which increase blood pressure.
Telma 20 mg selectively blocks the binding of angiotensin II to the AT1 receptor found in vascular smooth muscle and adrenal tissue. This action inhibits vasoconstriction and aldosterone secretion, leading to reduced peripheral vascular resistance and lowered blood pressure. Telmisartan has a very high affinity for the AT1 receptor, more than 3,000 times greater than for the AT2 receptor, which is not involved in cardiovascular regulation.
Unlike ACE inhibitors, Telma 20 mg does not inhibit ACE and therefore does not affect bradykinin metabolism, reducing the risk of bradykinin-related adverse effects such as cough.

Telma Tablet 20 mg is indicated for:
Treatment of essential hypertension in adults
Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease, including coronary heart disease, stroke, or peripheral arterial disease
Cardiovascular risk reduction in adults with type 2 diabetes mellitus with documented target organ damage
Dosage should be individualized according to patient response
Antihypertensive effect is dose-related within the range of 20–80 mg
Tablets may be administered once daily, with or without food
Most blood pressure reduction is observed within 2 weeks, with maximum effect achieved by 4 weeks
Telma 20 mg may be used alone or in combination with other antihypertensive agents
Initial therapy is not recommended in patients aged 75 years or older or in those with hepatic impairment
Concomitant use with NSAIDs may increase the risk of renal impairment and reduce antihypertensive efficacy
Co-administration with aliskiren should be avoided in patients with diabetes
Caution is advised when used with other drugs acting on the renin-angiotensin system
Known hypersensitivity to telmisartan or any component of the formulation
In hypertensive patients, commonly reported side effects include:
Sinus pain and congestion
Back pain
Diarrhea
In patients treated for cardiovascular risk reduction, side effects may include:
Intermittent claudication
Skin ulcer
Most adverse reactions are mild and transient.

Telma 20 mg is classified as Pregnancy Category C during the first trimester and Category D during the second and third trimesters
It should be discontinued immediately if pregnancy is detected
Breastfeeding should be discontinued or the drug stopped due to potential risk to the nursing infant
Avoid fetal or neonatal exposure
Monitor patients with renal or hepatic impairment closely
Hypotension may occur, especially in volume-depleted patients
Avoid concomitant use of ACE inhibitors and ARBs
Store below 30°C
Protect from light and high humidity
Keep out of reach of children.
